AR-11 Change of Address Form

This form must be filed at the address at the bottom of the form by non-US Citizens within 10 days of any change of address.

AR-11 Change of Address Form for Aliens Subject to Special Registration

Males, 16 years of age or older (born on or before November 15, 1986), who are nationals or citizens of Iran, Iraq, Libya, Syria, and Sudan, who were inspected and last admitted by the INS as a nonimmigrant on or before September 10, 2002, and who will remain in the United States at least until December 16, 2002 must file this form AR-11 and must do so on an annual basis or within 10 days of a change of address.

DS-156 and DS-157

These forms are required to apply for a non-immigrant visa at a US Embassy or Consulate. Consulates require that the electronic form of DS-156 be completed and submitted when applying for the visa.
